X线胸片与CT扫描在中央型肺癌诊断中的对比研究

目的对比分析X线胸片与CT扫描在中央型肺癌诊断中的价值。方法回顾性分析解放军第三二三医院2009年1月N2013年1月收治的经病理证实的98例中央型肺癌患者临床资料，进行CT扫描和X线胸片检查，比较两种方法的诊断结果，并对两种方法对患者支气管腔内、肺门区肿块及纵隔淋巴结肿大的情况进行对比分析。结果69例X线胸片检查者其中央型肺癌的诊出率为63．8％（44／69），93例CT扫描患者的诊出率高达91．4％（85／93），统计学分析显示CT扫描的诊出率明显优于X线胸片检查者（P〈0．01）。CT扫描对中央型肺癌支气管腔内、肺门区肿块及纵隔淋巴结肿大的显示清晰度分别为80．6％、77．4％和48．4％，显著高于X线胸片的显示情况（58．0％、53．6％、18．8％）（P〈0．05）。结论X线胸片和CT扫描均为中央型肺癌诊断的主要方法，但是CT扫描相对X线胸片而言．其准确性和灵敏度更高，对病变部位的清晰度显示也要优于X线胸片检查，具有更高的诊断价值。

Comparative analysis of the diagnostic value of X-ray and CT scan on central pulmonary carcinoma

Objective To compare the diagnostic value of X-ray and CT scan on central pulmonary carcinoma. Methods 98 patients with central pulmonary carcinoma confirmed by pathology in the 323rd Hospital of PLA from January 2009 to January 2013 were retrospectively analyzed. All patients were performed by X-ray or CT scanning. Results For the X-ray and CT images of the chest, the accuracy rate of X-my was 63.8% （44/69） for 69 patients, and the accuracy rate of CT scan was 91.4% （85/93） for 93 patients. There was statistical significance between two methods （P 〈 0.01）. In addition, the mass of bronchial lumen, hilus pulmonis and mediastinal lymph node enlargement was clearer using CT-scan （80.6%, 77.4% and 48.4%） than X-ray （58.0%, 53.6% and 18.8%） （P 〈 0.05）. Conclusion X-ray and CT scan are mainly methods for diagnosing central pulmonary carcinoma, and the CT scan has sharper and more sensitive resolution, and has a better diagnosis value of centrai pulmonary carcinoma.
