不稳定性心绞痛患者冠状动脉病变程度与外周血内皮祖细胞及高敏c反应蛋白的关系

摘    要:目的探讨不稳定性心绞痛患者冠状动脉病变程度与外周血内皮祖细胞(EPCs)及高敏C反应蛋白(hs—CRP)的相关性。方法选取2010年12月~2013年6月在南方医科大学附属东莞市石龙人民医院心血管内科住院治疗的不稳定性心绞痛患者55例为实验组,选择同期无冠状动脉狭窄患者30例为对照组。使用流式细胞仪测定外周血单个核细胞中CD34及CD133双阳性细胞EPCs的数量,并且检测每位患者hs—CRP水平。于住院后次日行冠状动脉造影,使用Gensini评分系统定量分析冠状动脉的病变程度。统计冠状动脉病变程度与外周血EPCs数量及hs—CRP的相关性。结果实验组与对照组EPCs分别为(30.95±5.50)、(58.66±4.45)个/10万.而实验组与对照组hs—CRP分别为(40.25±9.80)、(3.45±1.68)μmol/L,差异均有统计学意义(均P〈0.05);实验组内EPCs数量随着冠状动脉狭窄程度Gensini评分的增加亦随之升高(P〈0.05)。冠状动脉病变狭窄的程度与EPCs数量呈正相关(r=0.593,P〈0.01),而hs—CRP同冠状动脉病变程度无明显相关性(r=0.114,P〉0.05)。结论不稳定性心绞痛患者外周血EPCs数量减少,而hs—CRP则明显增加,但随冠状动脉病变程度加重,外周血EPCs数量增多,hs—CRP则没有明显改变,EPCs可以反映冠状动脉病变狭窄程度.而hs—CRP反映冠状动脉不稳定斑块的活动程度。

Abstract:Objective To investigate the correlation between endothelial progenitor cells (EPCs) from peripheral blood and hs-CRP with the degree of coronary artery lesions in patients with unstable angina. Methods 55 patients with unstable angina from December 2010 to June 2013 in Department of Cardiovascular, Dongguan Shilong People's Hospital of Southern Medical University were selected as experiment group, 30 cases without coronary artery stenosis in the same term were chosen as Control group. The number of circulating CD34+/CD133+ of EPCs from peripheral blood of the two groups were counted by flow cytometry and bs-CRP was measured. The degree of coronary artery lesions in the two groups was assessed with the Gensini scoring system. Then the correlation between the number of EPCs from peripheral blood and hs-CRP with the degree of coronary artery lesions were investigated. Results The number of EPCs of patients in experiment group and control group were (30.95±5.50), (58.66±4.45)/100 thousands respectively, but hs-CRP in experiment group and control group were (40.25±9.80), (3.45±1.68) μmol/L, the differences were all statistically significant (P 〈 0.05). The EPCs amount was increased with the degree of coronary artery stenosis and Gensini scores (P 〈 0.05), the positive correlation was found between EPCs amount and the degree of coronary artery stenosis (r = 0.593, P 〈0.01). There was no correlation between the degree of coronary artery lesions and hs-CRP (r = 0.114, P 〉 0.05). Conclusion There is depletion of the number of EPCs from peripheral blood and increasement of hs-CRP. There is increasement of EPCs amount with the exacerbation of coronary artery stenosis, but there is no change of hs-CRP. The EPCs can reflect the degree of coronary artery stenosis, hs-CRP can reflect the activity degree of coronary artery unstable plaque .
