摘    要:目的探讨使用国产止血带时,通过改进使用方法检测其止血效果及止血带损伤情况。方法以成人下肢手术为例制作弧形气压止血带保护套。选择施行择期下肢膝关节以下部位手术的成年患者100例,随机分为传统方法组、弧形气压止血带保护套组(改良方法组)各50例。传统方法组先用1张纱布垫缠绕肢体皮肤,缠上止血带,再用绷带缠在止血带上加以固定;改良方法组将保护套衬垫面缠在肢体上,在衬垫面上缠止血带,将反折面向上反折固定保护止血带,反折面两端的布带绑在保护套上。比较两组气囊止血带压力、止血效果及止血带损伤情况。结果两组气囊止血带压力比较,差异有统计学意义(P〈0.05),改良方法组气囊止血带压力低于传统方法组;两组充气时间比较,差异无统计学意义(P〉0.05);两组止血效果及止血带损伤情况比较,差异有统计学意义(P〈0.01),改良方法组手术时止血效果及减压后出现的止血带损伤进行综合判断,效果明显好于传统方法组。结论使用弧形气压止血带保护套能有效降低气囊止血带压力而不延长充气时间,达到止血效果及减少止血带损伤情况发生。

Abstract:Objective To explore the manufacturing and application of protecting jacket of vaulted air-pressure tourniquet. Methods The protecting jacket of vaulted air-pressure tourniquet was manufactured for operation on adult lower extremities. 100 adults undergoing lower extremity operations were equally randomized into two groups: the conventional haemostasis group and protecting jacket group. In the former group, one piece of gauze was used to wrap extremity skin, followed by wrapping with tourniquet and then by fixing it using bandage to wrap the tourniquet. In the latter, the protecting jacket was wrapped around extremity skin, followed by wrapping tourniquet on the jacket for protection of tourniquet. The pressure of air pressure tourniquet, haemostasis effect and injuries from tourniquet were compared between the two groups. Results There was statistical difference in pressure between the two groups (P〈0.05), the air pressure in the latter group lower than that in the former, but there was no difference in time for filling air (P〉0.05). In terms of the haemostasis effect and injuries by tourniquet after pressure delivery, the latter group was significantly better than the former (P〈0.01). Conclusion The protecting jacket of vaulted air-pressure tourniquet is effective for lowering the pressure of tourniquet and no lengthening the time for filling air so as to achieve the effect of haemostasis and alleviate injuries by tourniquet.
