空心双头螺纹钉加压固定骨瓣移植治疗青壮年股骨颈骨折

目的:探讨青壮年股骨颈骨折的治疗。方法:采用切开复位空心双头螺纹钉加压内固定缝匠肌骨瓣移植治疗青壮年股骨颈骨折,随访2~8年,平均4年。结果:43例新鲜骨折中1例不愈合股骨头坏死,1例骨折愈合后股骨头坏死;27例陈旧性骨折中,3例骨折不愈合股骨头坏死,2例愈合后股骨头坏死。愈合率94%,股骨头坏死率10%。结论:缝匠肌骨瓣移植可改善股骨头血供并使植骨以"活骨骨折愈合"的方式完成。陈旧性骨折股骨颈吸收可重建股骨颈,与空心双头螺纹加压钉有协同支撑固定的作用,是治疗青壮年股骨颈骨折的有效方法。

Treatment of the femoral neck fractures among young people by using the DHSN and the sartorial-pedicled ilium bone graft

Objective: Investigation of the treatment of the femoral neck fractureamong yotmg people.Methods: By cutting the double-heads compressive hollow screw for diaplasis intomal fixation of the sartorial-pedicled ilium bone graft to cure the femoral neck fracture among young people.Follow-up period is 2 to 8 years,average 4 years.Results: For the 43 cases fresh femoral neck fractures: one case fail to concrescence and then cause ostoonecrosis of frmoral head;one caseosteonecrosis of femoral head after the concrescence.For the 27 cases old fractures: 3 cases fail to concrescence and then cause osteonecrosis of femoral head;2 cases osteonecrosis of femoral head after the concrescence.Concrescence ratio is 94%,the osteonecrosis of femoral head ratio is 10%.Conclusion: Srtorius muscle pedicled iliac graft assisted with internal fixation can improve the blood supply of femoral head an made the bone transplantation is accomplished by the "vascularized bone graft union" method.For The old fracture,the absorb of the femoral neck can reestablish the femoral neck,and together with the assisted internal fixation of double heads hollow compression screw,this sure to be the effective methods forclue the femoral neck fracture among young persons.
