安速幽门螺杆菌快速检测在幽门螺杆菌现症感染诊断中的应用价值

摘    要:目的 探讨安速幽门螺杆菌快速检测试验(rapid detection of Helicobactor pylori testing,HP-RT)诊断幽门螺杆菌(Helicobactor pylori,HP)现症感染的临床应用价值.方法 对158例因上消化道症状进行胃镜检查的患者同时进行快速尿素酶试验(rapid urease test,RUT)、组织学染色(W-S银染)和C 14-呼气试验(C14-urea breath test,C14-UBT)和HP-RT,以RUT和W-S银染结果相一致为诊断HP 的金标准,评价HP-RT的临床诊断价值.采用SPSS 10.0统计软件对结果进行统计分析.结果 与金标准比较,HP-RT的敏感性为92.4%,特异性为87.0%,阳性预测值为97.7%,阴性预测值为87.0%,准确率为93.7%;与C14-UBT检测结果比较,HP-RT的敏感性为92.8%,特异性为86.2%,阳性预测值为93.8%,阴性预测值为88.7%,准确率为91.8%.HP-RT诊断HP的阳性率为54.4%,与金标准(58.9%)和C14-UBT(61.4%)比较,差异无统计学意义(x2=0.81,P>0.05).HP-RT和C14-UBT检测HP阳性率结果比较,差异亦无统计学意义(x2=0.03,P>0.05).结论 HP-RT具有较高的敏感性和特异性,在没有进行根除治疗的HP感染患者中能够较好的反应现症感染的情况,可用于HP现症感染的临床诊断检测.

The clinical diagnostic value of rapid detection of Helicobactor pylori testing in Helicobactor pylori pre- sent infection

Abstract:Objective To explore the clinical diagnostic value of rapid detection of Helicobactor py- lori testing(HP-RT) in Helicobactorpylori(HP) persent infection. Methods 158 cases patients with upper gus- trointestinal tract symptoms and detected by gastroscope were selected. They were detected by rapid urease test (RUT), histotogic dyeing (W-S staining) C14-urea breath test (C14-UBT) and HP-RT at the same time. The gold standard for HP was RUT and W-S silver stain results in agreement, which was used to evaluate clinical diag- nosis value of HP-RT. Results Compared with gold standard, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of HP-RT were 92.4%, 87.0%, 97.7%, 87.0% and 93.7% re- spectively. And compared with C14-UBT, these indexes were 92.8%, 86.2%, 93.8%, 88.7% and 91.8% respec- tively. The positive rate of HP-RT, gold standard and C14-UBT were 54.4%, 58.9% and 61.4%, there was no statistical significance in the differences of the three detection methods(x2= 0.81, P〉 0.05). There was statistical significance in difference of positive rate between HP-RT and C14-UBT results (X2= 0.03 ,P〉 0.05). Conclusion HP-RT has high sensitivity and specificity which can better reflect the present infection condition of HP pa- tients with no eradicate treatment. HP-RT could be used for clinical diagnosis of HP present infection.
