梗阻性黄疸的超声诊断

目的:探讨梗阻性黄疸的超声表现与其病理性质之间相关性,以提高超声对梗阻病因的诊斯符合率.方法:对63例梗阻性黄疸进行超声检查,声像图特点与其病理结果进行对照分析.结果:胆总管结石诊断符合率90.9%(30/33);先天性胆总管囊肿(3/3);胆总管内蛔虫(2/2);胆总管疤痕性狭窄(1/1);胆管癌83.3%(5/6);壶腹周围癌77.8%(7/9);胰头癌87.5%(7/8);十二指肠乳头状癌(0/1).结论:超声能动态观察病变所在部位及其与周围的关系,是梗阻性黄疸诊断和鉴别诊断的有效检查方法.

On ultrasonography in obstructive jaundice

Objective To evaluate the diagnostic accuracy of ultrasonography compared with its pathology in obstructive jaundice. Methods 63 patients with obstructive jaundice were examined by means of comparing image features and pathological diagnosis. Results The accuracy rate was 90. 9% (30/33) in diagnosing choledochal duct calculus, while it appeared 100% in congenital cyst of choledochal duct, infestation with a parasitic roundworm, and choledochal duct stenosis caused by scars respectively. The accuracy rate was also high in diagnosis of other cancers such as carcinoma of pancreas head (87.5%) and cholangiocarcinoma(83.3%). Conclusion The pathological changes can be observed dynamically by ultrasonography and it is an effective method to distinguish the different diseases in obstructive jaundice.
