麦角新碱冠状动脉痉挛激发试验的临床应用

目的：通过左右冠状动脉选择性地注入马来酸麦角新碱激发冠状动脉痉挛，经冠状动脉造影来评价冠状动脉痉挛的有无及程度。方法：对临床怀疑冠状动脉痉挛而冠状动脉造影未见显著器质性狭窄者，左右冠状动脉分别先后注入０．０２ｍｇ的马来酸麦角新碱，３分钟后用冠状动脉造影来评价冠状动脉的痉挛。结果：９例患者中５例阳性，显示了麦角新碱冠状动脉痉挛激发试验对冠心病心绞痛和急性心肌梗死发病原因判定的价值。结论：此方法对以冠状动脉痉挛为主的冠心病的诊断具有较高的敏感性，可用于指导用药；虽有发生严重并发症的可能性，但只要技术熟练，操作谨慎是切实可行的。

Clinical Use of Ergonovine in the Provocative Test of Coronary Artery Spasm

Objective:Following coronary arteriography,the provocative test was performed using selective infusion of ergonovine maleate into left and right coronary arteries.The incidence and degree of coronary artery spasm were observed by coronary arteriograpy. Methods:Patients suspected to have coronary artery spasm but no stenosis on coronary arteriography were studied.Ergonovine maleate was successively infused respectively into the left and right coronary artery in a dosage of 0 02 mg.Coronary artery spasm was evaluated by coronary arteriography three minutes later. Results:Five of the 9 cases were positive,which indicated that the provocative test of ergonovine was of great value in exploration of the etiology of angina pectoris and acute myocardial infarction. Conclusions:This method is highly sensitive for diagnosing coronary artery disease in which coronary artery spasm accounts for the predominant factor,and is also useful to guide medication.So long as the operator is skilled and cautious,in spite of the possible danger of complications,it is practicable.
