Investigation on the Current Status of Blood Glucose Management in Patients with Diabetes Mellitus after Orthopedic Surgery

Objective To investigate and analyze the blood glucose management behavior of patients with diabetes after orthopedic surgery after discharge,to understand the status of self-management of blood glucose in patients with diabetes during the perioperative period,and to provide guidance for carrying out continuous nursing care for patients with diabetes during the perioperative period.Methods A telephone questionnaire survey was conducted on 223 patients with diabetes mellitus after surgery in the department of orthopedics of a General Hospital of Fuzhou City from June 2018 to June 2019,7 d,14 d and 1 month after discharge.Results 223 cases of diabetic patients were 124 males and 99 females,aged 22-82 years old,with an average age of(57.56±15.98)years;education level≤122 cases of primary school,79 cases of junior high school and high school,≥22 cases of specialty;farmer 48 cases,51 cases of working staff,124 cases of retired and unemployed persons;89 cases of newly diagnosed diabetes,134 cases of non-newly diagnosed diabetes;7 d after discharge from hospital,diet management,exercise management,blood glucose monitoring,drug treatment,and endocrine follow-up average scores were higher than the 14 d and 30 d average scores after discharge,the difference was statistically significant(P<0.05),and the scores of blood glucose management 14 days after discharge were higher than the 30-day average scores after discharge,the difference was statistically significant(P<0.05).There was no correlation between gender and blood glucose self-care behavior(P>0.05).The older the age,the higher the blood glucose self-care behavior score.The education level was the highest score from junior high school to high school.The self-care behavior score of elementary school and below was the lowest.Retired people of the self-protection behavior scores the highest,the in-service personnel scores in the middle,and the lowest was the farmers.In particular,the self-care behavior scores of first-diagnosed diabetic patients were lower after discharge;age,education level,occupation,and whether they were newly diagnosed with diabetes were correlated with self-care behavior scores(P<0.05).Patients with diabetes mellitus after orthopedics surgery accounted for 66.8%of their demand for blood glucose education after discharge.Conclusion The non-endocrinology department should strengthen the education of diabetes related to the discharge management of patients with diabetes during the perioperative period,especially for patients with diabetes for the first time,to improve their blood glucose self-management ability.
