支链氨基酸代谢与妊娠期糖尿病的相关性研究

摘    要:目的通过检测妊娠期糖尿病(GDM)患者的外周血中支链氨基酸的含量,探讨支链氨基酸代谢与机体糖脂代谢的相关关系。方法选择该院2018年1月—2019年1月期间GDM孕妇56例、血糖正常孕妇58名分别作为GDM组及正常对照组。评估两组孕妇外周血的BCAAs含量、糖代谢以及脂质代谢指标的水平差异,及BCAAs含量与糖脂代谢紊乱的相关关系。结果GDM组血清中亮氨酸与异亮氨酸的总量(84.80±14.64)μmol/L、缬氨酸的含量(115.88±18.19)μmol/L均显著高于正常组(74.33±11.92)μmol/L、(105.38±17.61)μmol/L,差异有统计学意义(t=4.179、3.130,P<0.05);GDM组外周血糖代谢指标糖化血红蛋白、空腹胰岛素以及胰岛素抵抗指数的水平均显著高于正常对照组;GDM组外周血中脂质代谢指标总胆固醇、高密度脂蛋白、低密度脂蛋白的含量显著低于正常对照组,甘油三酯的含量显著高于正常对照组,差异有统计学意义(P<0.05)。相关性分析表明,GDM组的支链氨基酸含量与机体糖脂代谢紊乱程度呈正相关。结论GDM组孕妇存在支链氨基酸含量升高的现象,可能是促进机体糖脂代谢紊乱的重要因素。

Abstract:Objective To investigate the relationship between branched-chain amino acid metabolism and body glucose and lipid metabolism by detecting the content of branched-chain amino acids in the peripheral blood of patients with gestational diabetes(GDM).Methods Fifty-six pregnant women with GDM and 58 pregnant women with normal blood glucose from January 2018 to January 2019 in the hospital were selected as the GDM group and the normal control group.To evaluate the differences in the levels of BCAAs,glucose metabolism and lipid metabolism in peripheral blood of pregnant women between the two groups,and the correlation between BCAAs content and glucose and lipid metabolism disorders.Results The total contents of leucine and isoleucine(84.80±14.64)μmol/L and the content of valine(115.88±18.19)μmol/L in serum of GDM group were significantly higher than those of normal group(74.33±11.92)μmol/L and(105.38±17.61)μmol/L,the difference was statistically significant(t=4.179,3.130,P<0.05);the levels of peripheral blood glucose metabolism indexes glycosylated hemoglobin,fasting insulin,and insulin resistance index in the GDM group were significantly higher than those in the normal group;the levels of total cholesterol,high-density lipoprotein,and low-density lipoprotein in the peripheral blood of the GDM group were significantly lower than those of the normal control group,and the content of triglycerides was significantly higher than that of the normal control group,the difference was statistically significant(P<0.05).Analysis showed that the content of branched-chain amino acids in the GDM group was positively correlated with the degree of the body's glucose and lipid metabolism disorder.Conclusion There is an increase in the content of branched chain amino acids in pregnant women in the GDM group,which may be an important factor in promoting the body's glucose and lipid metabolism disorder.
