摘    要:目的:通过动物实验研究胃液及十二指肠内容物反流对食管肿瘤发生过程的影响。方法:通过不同手术方式制作3种动物模型:胃食管反流(G)组,单纯十二指肠食管反流(D)组,十二指肠,胃混合食管反流(DG组)及无反流对照组(C组)。术后均注射食管致癌剂甲基戊基亚硝胺(MANA),于26周取出食管进行大体及光镜下形态学观察。结果:各反流组出现严重食管粘膜损害,病变较对照组明显加重且发生率高,病变严重程度D组〉D

Abstract:Objective: To investigate the effect of different reflux agent on esophageal tumorgenesis in rats. Methods: Three kinds of esophageal reflux model and control group(C) were made by operations: gastroesophageal reflux(G), duodenoesophageal reflux(D) and duodenogastroesophageal reflux(DG).All rats were given carcinogen (methyl n amyl nitrosamine) and their esophagi were taken 26 weeks after operation and studied by morphologic observation. Results: Most rats in reflux groups displayed evidence of severe esophageal mucosal injury .The rates and severity of changes in reflux groups were obviously greater than that in control group. Among the reflux groups,esophageal injury was the most severe in D group,then DG group and the least in G group.The incidences of papillomas were D 40.9%,DG 21.7%,G 5.6%,C 0%.The differences between 4 groups were significant. Esophageal columnar metaplasia were found in 7 and dysplasia in 6 in D and DG group,and none were found in G and C group. Conclusion: Both gastric juice and duodenal contents, refluxing into esophageal alone or simultaneously, can promote tumourgenesis of MANA in rats esophagi. The effect of duodenal contents is stronger than gastric juice. It may play an important role in tumourgenesis of reflux esophagitis.
