2型糖尿病患者血尿酸水平与尿清蛋白排泄的关系

目的探讨2型糖尿病(T2DM)患者血尿酸(SUA)水平与尿清蛋白排泄(UAE)之间的关系。方法 2型糖尿病患者280例,男164例,女116例。测定空腹血糖(FPG),总胆固醇(TC),三酰甘油(TG),血尿酸(SUA),血肌酐(SCr)和24 h尿清蛋白排泄量(24 h-UAE),记录血压和计算体重指数(BMI)。结果①男性血尿酸水平(276.93±87.51)mmol/L,高于女性(244.72±85.83)mmol/L,P0.05;②正常、微量及大量清蛋白尿组的SUA分别为:(243.58±63.93)mmol/L、(261.47±89.11)mmol/L、(334.23±113.82)mmol/L。与正常清蛋白尿组和微量清蛋白尿组比较,大量清蛋白尿组SUA水平明显升高,同时伴收缩压、BMI、SCr的增高和病程的延长,P均0.05。结论 2型糖尿病患者血尿酸升高与尿清蛋白排泄有关,高尿酸血症可能参与糖尿病肾损害的发生。

Relationship between serum uric acid and urinary albumin excretion in patients with type 2 diabetesmellitus

Objective To investigate the relationship between serum uric acid(SUA)and urinary albumin excretion(UAE) in patients with type 2 diabetes mellitus(T2DM).Methods The clinical data of 280 patients with T2DM,including 164 males and 116 females.The following indices were detected: Fasting plasma glucose(FPG),cholesterol(TC),triglyceride(TG),serum uric acid(SUA),serum creatinine(SCr),24h-urinary albumin excretion(24h-UAE).Blood pressure(BP) and body mass index(BMI) were recorded.Results(1) Serum uric acid of males(276.93±87.51)mmol/L is higher than that of females(244.72±85.83)mmol/L,P<0.05;(2)Serum uric acid of patients with normoalbuminuria,microalbuminuria and microalbuminuria were(243.58±63.93)mmol/L,(261.47±89.11)mmol/L,(334.23±113.82)mmol/L.Comparing with the patients with normoalbuminuria and microalbuminuria,serum uric acid in patients with macroalbuminuria significantly increased.At the same time,systolic blood pressure、BMI、SCr were also increased and the course of disease was prolonged.Conclusion The increased serum uric acid is related with urinary albumin excretion.High serum uric acid maybe participate in the occurrence of diabetic nephropathy.
