摘    要:目的探讨持续微泵联合应用阿托品与氯解磷定对重度急性有机磷农药中毒的救治疗效。方法将60例重度急性有机磷农药中毒患者随机分成持续微泵联合应用阿托品与氯解磷定组(试验组)与间歇静脉注射阿托品与氯解磷定组(对照组),比较2组间阿托品化时阿托品和氯解磷定用量、阿托品化及血浆胆碱酯酶活性(CHE)恢复时间、阿托品化时急性生理学和慢性健康状况Ⅱ评分(APACHEII评分)及IgA水平、病死率等指标。结果试验组救治过程中的阿托品和氯解磷定用量、阿托品化及胆碱酯酶活性恢复时间、阿托品化时APACHEII评分及IgA水平均优于对照组,病死率亦低于对照组(P0.05)。结论持续微泵联合应用阿托品与氯解磷定是提高救治重度急性有机磷农药中毒的有效方法。

Abstract:Objective To observe the efficacy of continuous micro-pump combined atropine and pralidoxime chloride on the treatment of severe acute organophosphorus pesticide poisoning(AOPP).Methods The total of 60 patients with severe AOPP were randomly divided into continuous micro-pump atropine and pralidoxime chloride group(test group) and intermittent injection of atropine and pralidoxime chloride group(control group).The dosage of atropine and pralidoxime chloride,the time of atropinization and CHE recovery,the APACHEII score and IgA level during atropinization,and mortality were measured.Results Compared with patients in control group,the time of atropinization and CHE recovery,the amount of atropine and pralidoxime chloride when occurred atropinization,IgA level,APACHEII score in test group showed a statistically significant therapeutic effect(P<0.05),and the mortality of test group was lower than that of control group(P<0.05).Conclusion Continuous micro-pump combined atropine and pralidoxime chloride is a more effective way in the treatment of severe acute organophosphorus pesticide poisoning.
