TRPV1受体在胃食管反流病患者食管黏膜的表达及其与症状的关系

目的研究瞬时感受器电位香草酸受体1(TRPV1,也称为辣椒素受体)在胃食管反流病患者食管黏膜组织的表达及其与症状的关系。方法 GERD患者54例(RE患者28例、NERD患者26例)及对照组25例进行电子胃镜检查时,在胃食管连接处上方3 cm处食管活检2块黏膜组织行HE染色和TRPV1免疫组化检查,并使用RDQ问卷对所有受试者进行症状评估。结果 RE组TRPV1受体表达的光密度(OD)值为为0.022 2±0.0029 9,NERD组OD值为0.020 70±0.001 77,对照组OD值为0.005 5±0.000 39,GERD患者食管黏膜TRPV1受体的表达显著高于对照组,NERD组和RE组之间的TRPV1受体的表达差异无统计学意义;TRPV1受体的表达与患者症状程度相关。结论 GERD患者食管黏膜TRPV1受体表达增加,可能与患者症状的产生有关。

Correlation between the expression of TRPV1 in the esophageal mucosa and symptoms of patients with gastroesophageal reflux disease

Objective To study the expression of the transient receptor potential vanilloid type l(TRPV1,also referred as the capsaicin receptor) in the esophageal mucosa of patients with gastro-esophageal reflux disease(GERD) and analyse the relationship between TRPV1 and the symptom of the patients.Methods 54 GERD patients(28 RE patients,26 NERD patients) and 25 healthy controls were recruited into the study.During the endoscopic examination,two biopsies were taken from esophagus at a fixed position 3cm above the gastro-esophageal junction in all subjects.Biopsies were used for H&E histological staining and immunohistochemical staining using specific antibodies.Symptoms of patients were evaluated through the reflux disease diagnostic questionnaire(RDQ).Results The optical densities of the TRPV1 protein levels were 0.022 2±0.0029 9 in the esophageal mucosa of RE patients,0.020 7±0.001 77 in NERD patients,and 0.005 5±0.000 39 in controls.Compared with the controls,the expression of TRPV1 was markedly higher in the esophageal mucosa of GERD patients,but those were similar in RE and NERD patients.The expression was correlated with the symptom score.Conclusion Increased TRPV1 expression in the esophageal mucosa may contribute to symptoms in GERD patients.
