普迪思缝线治疗髌骨骨折

摘    要:目的: 探讨普迪思可吸收线固定的生物力学性能及其治疗髌骨骨折的效果。方法: 以WDW305型电子万能试验机施行PDS0#和PDS1#的拉力试验; 本组病例 30例, 直视下解剖复位, 巾钳固定。用直径 1. 5mm的克氏针, 在骨折远近端或与折线垂直的两侧各做横行侧位髌骨前后径中心或稍偏后钻两孔, 分别穿入 2道PDS0#或PDS1#可吸收线, 近端的内侧头线和远端的外侧头线、近端的外侧头线与远端的内侧头线分别在髌骨骨折前缘做交叉张力带打结固定, 另在近端的内侧头线和远端的内侧头线、近端的外侧头线与远端的外侧头线做髌骨骨折内缘、外缘压力带打结固定。结果: 通过拉力实验, PDS0#单股的最大拉力为71. 50N,PDS1#单股的最大拉力为 97. 48N, 测试中达到最大拉力时, 其长度为原长度的 3倍; 两组病例全部获得随访, 平均骨折愈合时间为 2个月, 未发生折断再移位, 普迪思线治疗组, 优 21例, 良 6例, 可 3例。结论: 普迪思可吸收张力带治疗髌骨骨折的生物力学实验表明, 具有较好的力学性能和弹性, 而且经济便宜, 有利于推广应用。

Abstract:Objective:To explore patellar fracture with absorbable Polyloxanone (PDS) biomechanical characteristics and clinical outcome.Method: The tensile strength test of PDS 0 # and PDS 1 # were making in WDW305 electro universal tester.Thirty cases of patelllar fracture were performed in the operation,with diameter 1.5mm needle drilling 2 on the patellar fracture end,through 2 strand absorbable PDS 0 # or PDS 1 # fixation.Result: Tensile strength testing shown among a strand PDS 0 # the biggest tensile was 71.5N,PDS 1 # was 97.48N.When test the biggest tensile,its length more than 3 times initial.All patients were followed up,mean fracture healing were 2 months,no broken and local site was bad, excellent 21 cases,good 6 cases, fair 3 cases. Conclusion: Testing of biomechanics showed that absorbable PDS tension band treated patellar fracture have better charateristics of mechanics and elasticity.Econoiny is cheaper,it is beneficial to patellar fracture healing in clinical application.
