腰椎椎体后缘软骨结节的影像学诊断

目的：总结腰椎椎体后缘软骨结节所致腰椎管狭窄症的临床、影像学表现及诊断要点．探讨有关的发病因素。方法：对32例腰椎椎体后缘软骨结节所致腰椎管狭窄症的临床表现、影像学特征进行分析。结果：32例均有间歇性跛行，多以腰腿痛为主诉就诊，9例推拿按摩后症状加重。X线腰椎侧位片见病变椎体（以腰4、5为主）后下（或上）角有骨缺损及该区有游离小骨块突入椎管内；典型的影像学改变为：（1）椎体后上缘类圆形骨质缺损，边缘较致密硬化；（2）缺损后方有一骨块突入椎管（游离或部分与椎体相连）；（3）硬膜囊及神经根受压，可伴有椎间盘突出。结论：CT有特征性表现，能为制订治疗方案提供可靠依据。

Imaging diagnosis of lumbar posterior marginal intraosseous cartilaginous node

Objective:To summarize the clinical and imaging characteristics of lumbar spinal canal stenosis caused by lumbar posterior marginal intraosseous cartilaginous node(LPMN),and to discuss its related etiological factors. Methods:The clinical manifestations and imaging characteristics of 32 cases with lumbar spinal canal stenosis caused by LPMN were analysed. Results:The typical imaging findings included:(1)focal defect in the posterior superior margin with sclerotic edge;(2)the defect foci which appeared the bony fragment sticking out the spinal canal;(3)compression of the dural sac and nerve root,mostly with lumbar discprotrusion.Conclusion:CT examniation is typical,and can provide a valuabe information for planning the treatment protocol.
