重型颅脑损伤患者持续性气道湿化在气管切开护理中的应用

目的：探讨重型颅脑损伤患者气管切开后起到持续湿化的效果。方法：选择笔者所在医院2008年10月～2010年10月收治的重型颅脑损伤患者63例,随机分为研究组32例和对照组31例,研究组患者给予氧气雾化持续气道湿化,对照组患者给予间断气道湿化,观察两组气道湿化效果及呼吸、心率、血氧饱和度变化。结果：研究组患者刺激性咳嗽、气道黏膜出血、痰阻、肺部感染发生率（6.3%、6.3%、3.1%、9.4%）明显少于对照组（25.8%、19.4%、25.8%、32.2%）,两组差异有统计学意义（χ2=17.14,14.69,19.96,19.13,P〈0.05）。研究组心率、呼吸频率低于对照组（86±10）/min,（18±3）/min vs（109±11）/min,（25±7）/min],氧分压、血氧饱和度高于对照组（12.1±1.8）kPa,（95.8±4.0）%vs（8.6±3.0）kPa,（91.2±5.7）%],两组比较差异有统计学意义（t=11.37、7.93、6.28、5.39,P〈0.05）。结论：持续氧气雾化湿化气道用于重型颅脑损伤气管切开患者,排痰效果好,肺部感染少,血氧饱和度高,值得应用。
