家庭无创正压通气结合家庭氧疗长期治疗COPD合并呼吸衰竭

目的：观察家庭无创正压通气结合家庭氧疗长期治疗COPD合并呼吸衰竭患者的临床疗效。方法：将64例COPD合并呼吸衰竭患者分为治疗组（33例）和对照组（31例）,在常规治疗的基础上治疗组进行长期家庭无创正压通气和家庭氧疗治疗;对照组进行长期家庭氧疗。结果：两组患者血气分析指标均明显改善,差异有统计学意义（均P〈0.05）;其中,治疗组患者PaCO2、PaO2改善情况优于对照组,差异有统计学意义（P〈0.05）,pH值组间差异无统计学意义（P〉0.05）。肺功能治疗组无明显变化,对照组明显下降,组间比较,差异有统计学意义（P〈0.05）。病情恶化次数、年医疗费用支出、平均住院天数,治疗组优于对照组,差异有统计学意义（P〈0.05）。结论：长期家庭无创正压通气结合家庭氧疗可以改善COPD合并呼吸衰竭患者的血气指标,延缓肺功能的进行性恶化,减少COPD急性加重的频率及医疗支出。

Clinical observation of long-term family noninvasive positive pressure ventilation plus oxygen therapy in the treatment of COPD combined respiratory failure

Objective： To observe the effect of long-term family noninvasive positive pressure ventilation plus oxygen therapy in the treatment of COPD combined respiratory failure.Methods： 64 patients were randomly divided into treatment group（33 cases） and control group（31 cases）.On the basis of routine treatment,the treatment group was received NIPPV and LTOT treatment;the control group was received LTOT treatment.Results： The blood gas indicators of the two groups were obviously improved with a statistically significant difference（P0.05）;PaCO2,PaO2 of the treatment group were better than those of the control group（P0.05）,the difference of pH between the two groups was no significant difference（P0.05）.Lung function had no obvious change in the treatment group while the control group dropped significantly,and the difference was statistically significant（P0.05）.In terms of deterioration in frequency,medical expense,average hospitalization days,the difference was statistically significant in the two groups（P0.05）.Conclusion： The treatment of long-term family noninvasive positive pressure ventilation plus oxygen therapy can improve the blood gas analysis,delaying the worsening of lung function,and reducing the frequency and medical expenses.
