抗肌球蛋白重链和轻链抗体在扩张型心肌病与冠心病鉴别诊断中的价值

目的　旨在探讨抗肌球蛋白重链和轻链抗体，在扩张型心肌病（DCM）和冠心病中鉴别诊断的价值。方法对21例DCM患者，14例冠心病患者及正常健康组18例，进行临床免疫学检测，应用ELISA法，免疫转印法检测数值予以分析。结果ELISA法DCM组阳性率43．9％，冠心病组28．6％，正常对照组均为阴性。免疫转印法21例的DCM患者抗肌球蛋白重链抗体（200KD）10例阳性，阳性率47.6％，冠心组抗肌球蛋白轻链抗体（27．SKD）阳性4例，阳性率28．6％，PBS替代试验均为阴性。结论抗心肌肌球蛋白重链和轻链的抗体检测有助于DCM和冠心病的鉴别诊断。

The value of antimyosia heavy chain and light chain In the differential diagnosis between dilated cardiomyopathy and coronary heart disease

Ohjective To explore tHe value of antimyosia heavy chain and light chain in the dif-ferential diagnosis between DCM and CHD. Methods Clinical immunological detection was performed in 21 patiients with dilated cardiomyopathy (DCM),l4 patients with coronary heart disease (CHD) andl8 healthy people. ELISA and lmmunoblot protein footprint were used in the analysis. Results UnderELISA, the the positive rate of DCM patients was 43. 9%, that of CHD patients was 28. 6% and that of thecontrols was negaive. While under protein footprint, the positive rate of antimyosin heayy chain(200KD) in DCM patients was 47. 6%, that of antimyosin light chain (27. 5HD) in CHD patients was28. 6 94. PBS Suhstitution tests were all negative. Conclusion Detection of antimyocardial myosinheavy chain and light chain is useful in the differential diagnosis between DCM and CHD.
