老年肺结核CT诊断

目的：探讨老年肺结核的CT表现并评估CT的诊断价值。方法：回顾性分析经手术病理、纤支镜、痰菌培养所证实的56例老年肺结核的CT表现，常规CT平扫和增强扫描，病灶部位2mm薄层扫描。结果：肺内原发病灶56例，结核性空洞38例，干酪性肺炎40例，结核性胸膜炎20例，支气管旁、肺门及纵隔淋巴结肿大17例，粟粒性肺结核5例。老年肺结核的CT征象多表现为形态不一，肿块特征不明显，病变易播散，病灶数目较多，易发生钙化和少增强效应，易合并结核性胸膜炎和心包炎。结论：CT检查在老年肺结核诊断中起重要作用，为老年肺结核的诊断和鉴别诊断提供可靠的依据。

CT diagnosis of the elderly patients with pulmonary tuberculosis

Objective： To discuss CT findings and evaluate diagnostic value of the elderly patients with pulmonary tuberculosis.Methods： CT findings in 56 eases of the elderly patients with pulmonary tuberculosis confirmed by surgery, pathology, endoscopy, tubercle bacillus culture were retrospectively analyzed. All cases were scanned by CT and enhanced study was done, the location of lesions slice thickness：2mm. Among them, males（39 cases）, female （ 17 cases）. Resuits：The primary lesions of pulmonary tuberculosis（ 56 cases）, tuberculous cavity（ 38 cases ）, caseous pueumonia（ 40 cases ）, tuberculous pleurity（ 20 cases）, bronchi , hilar and mediastimal enlarged lymph nedes（17 cases）, miliary tuberculosis （5 cases）. CT features were as follows： different configuration, mass was not common, apt to disseminated, multiple lesions and more calcification, less enhanced effect, often complicated with tuberculous pleurity and pericarditis. Conclusion： CT is an importsnt method for diagnosis of elderly patients with pulmonary tuberculosis, can provide more information for diagnosis and differential diagnosis of the elderly patients with pulmonary tuberculosis.
