肌萎缩侧索硬化症和脊髓型颈椎病的误诊和相互关系的探讨

目的:通过对以往诊断为肌萎缩侧索硬化症(amyotrophiclateralsclerosis,ALS)的48例患者做回顾性重新检查,以探讨肌萎缩侧索硬化症(amyotrophiclateralsclerosis,ALS)和脊髓型颈椎病(cervicalspondylotic myelopathty,CSM)的相互关系。方法:对误诊的48例ALS的患者进行详细的神经系统、临床症状、体征、CT、MRI及神经电生理学检查,同时行相关因素分析。结果:误诊的48例ALS的患者,其中22例患者确诊CSM,18例患者CSM和ALS共存,8例维持原诊断。结论:神经电生理检查是CSM和ALS鉴别的关键;CSM和ALS共存不是偶然的,CSM可能是ALS疾病的重要病因。

Misdiagnosis and relationship of amyotrophic lateral sclerosis and cervical spondylotic myelopathty

Objective 48 patients that had been diagnosed as amyotrophic lateral sclerosis(ALS) by neurologists were reexamined to evaluate the relationship of ALS and cervical spondylotic myelopathty(CSM).Method Clinical manifestation,objective sign and examination of nervous system,CT,MRI and EMG were conducted on 48 cases diagnosed as ALS.Results Among the 48 patients,22 cases were diagnosed as CSM,18 cases coexisted CSM and ALS,and 8 cases were ALS.Conclusion EMG,CT and MRI are valuable in the diagnosis of CSM and ALS,while EMG is thought to be the most important.CSM maybe one of the important etiopathogenisis of ALS.
