摘    要:【目的】探讨1μg/ml大蒜稀释喷液、1%盐酸特比萘芬喷剂、2%酮康唑洗剂对海南岛某部新兵集训中皮肤真菌病的预防作用。【方法】选择某部2008年12月所招新兵,随机抽样健康者920人,随机分成4组。首先通过沙堡氏琼脂培养基(SDA)倍比稀释法测定大蒜稀释液针对红色毛癣菌与须癣毛癣菌的最低抑真菌浓度。按照两者中较高的最低抑真菌浓度配制大蒜稀释液,用于第1组研究对象的手、足、腹股沟,1%盐酸特比萘芬喷剂、2%酮康唑洗剂、蒸馏水分别用于第2、3、4组研究对象的手、足、腹股沟。观察受试试剂在新兵训练3个月中,预防皮肤真菌病的效果。对在受试3个月中所试部位出现皮肤病的新兵,将其受损皮屑或分泌物用沙堡氏琼脂培养基进行真菌培养,分析3种外用制剂对皮肤真菌病的预防效果。【结果】采用沙堡氏琼脂培养基倍比稀释法测定大蒜稀释液针对红色毛癣菌的最低抑真菌浓度(MIC)为0.25μg/ml,针对须癣毛癣菌的最低抑真菌浓度(MIC)为1μg/ml;第1组与第4组预防效果差异有统计学意义,第1组优于第4组(P<0.05);1μg/ml大蒜稀释喷液与1%盐酸特比萘芬喷剂、2%酮康唑洗剂在某部新兵集训中预防皮肤真菌病的效果相近(P>0.05)。【结论】一定浓度的大蒜稀释液可抑制红色毛癣菌、须癣毛癣菌的生长;1μg/ml大蒜稀释喷液、1%盐酸特比萘芬喷剂和2%酮康唑洗剂均能有效预防新兵集训中皮肤真菌病的发生,其携带方便,使用简单安全,可以在部队集训或战争中推广。

Abstract:【Objective】Invest the prophylactic effect of a 1μg/ml garlic spray,a 1% Terbinafine spray,a 2% Ketoconazole lotion against recruits'dematomycosis in Hainan island.【Methods】920 healthy recruits were confessed from a troop enrolledin December,2008.They were divided into four groups.First,the MIC(minimal inhibitory fungal concentration) of garlic liquid against Trichophyton rubrum and Trichophyton mentagrophytes were determined by SDA(Sabouraud's dextrose Ager) dilution referenced the M38-A proposal established by NCCLS.Second,garlic liquid was prepared according to the higher one of MIC.The garlic spray was externally used for the first group everyday on hands,feet and inguinal grooves.A 1% Terbinafine spray,a 2% Ketoconazole lotion and distilled water were externally used differetly for the second,third and forth group everyday on the same positions for three months.The effects of the four preparations preventing dematomycosis were observed by concurrent randomized control signal blind t吧rial when recruits were training.The damaged scurf or secretion from the recruits' positions examined in the three month were cultivated in SDA.At last,the statistical analysis was made by χ^2 test(Chi-square test).【Results】The MIC of garlic liquid against Trichophyton rubrum determined by SDA was 0.25μg/ml.The MIC of garlic liquid against Trichophyton mentagrophytes 【Conclusion】A garlic liquid of a particular concentration may restrain the growth of Trichophyton rubrum and Tricophyton mentagrophytes.A 1/ml garlic spray,a 1% Terbinafine spray and a 2% Ketoconazole lotion may prevent dematomycosis when recruits are training.These preparations may be taken easily and be used safely.It is worthy that the three preparations are used extensively to prevent dematomycosis when troops are training or in wars.
