肥厚型心肌病69例临床分析

目的分析肥厚型心肌病(HCM)的临床特点,提高对疾病的诊断及鉴别诊断水平。方法回顾分析69例患者的临床表现、心电图、超声心动图检查结果并进行统计学分析。结果HCM临床表现复杂多样,以劳力性呼吸困难多见,有26例(37.4%)。55例有心电图结果中,53例(97.2%)心电图异常,其中45例(82.6%)ST段压低,31例(57.8%)左胸导联T波倒置,27例(48.6%)左心室肥厚,11例(20.2%)异常Q波;超声心动图检查示非对称性54例(78.4%),对称性15例(21.6%),心尖肥厚型5例(6.5%),梗阻型12例(18.0%),非梗阻型57例(82.7%)。结论HCM诊断主要依靠超声心动图,结合临床表现与心电图改变。

Clinical characteristics of hypertrophic cardiomyopathy(69 cases)

Objective Analyzing of the clinical characteristics of 139 patients,to improve the diagnosis and differential diagnosis for hypertrophic cardiomyopathy.Methods 69 patients were retrosprctively analysed about their clinical symptom,electrocardiogram and echocardiographic features.Results Main symptom:26 cases(37.4%)had the effort dyspnea;ECG:45 cases(82.6%)of the ST segment depression,31 cases(57.8%)of the T wave inversion,27 cases(48.6%)of the left ventricular hypertrophy and 11 cases(20.2%)of the abnormal Q wave;echocardiogram:55 cases(78.4%)of the asymmetric hypertrophy,57 cases(82.7%)of the non-obstructive hypertrophy and 5 cases(6.5%)of the apical hypertrophy.Conclusions HCM could be suspected by the clinic symptom and electrocardiogram,echocardiogram is a reliable diagnostic approach.
