骨化性纤维瘤和骨纤维异常增殖症的影像学比较

摘    要:目的:提高对骨化性纤维瘤和骨纤维异常增殖症影像学诊断的准确性,合理选择影像学检查手段。方法:通过对50例经病理证实的骨化性纤维瘤(OF)和骨纤维异常增殖症(FD)的影像学比较分析,探讨其影像学表现的差别。结果:在24例OF中,16例发生于颅(颌)面骨髓腔,表现为类圆形膨胀性生长、边界清楚的不均匀高密度影,病变范围多局限于单骨;6例发生于胫骨前侧皮质内,沿其长轴纵行偏心膨胀性生长。在26例FD中,颅(颌)面骨FD多表现为边界不清的弥漫性骨髓腔闭塞膨大和磨玻璃改变,在四肢骨多为髓腔内囊状磨玻璃改变,多数病变范围较广泛,少数较局限并伴硬化边。结论:OF和FD的影像学鉴别点在于病变的部位、边界以及累及范围,影像学检查应以X线平片为主,选择性辅以其他影像学检查手段。

Abstract:Objective: To improve imaging diagnostic accuracy of ossifying fibroma(OF) and fibrous dysplasia(FD) of bone as well as correct imaging examination. Methods: A comparative study of imaging manifestations on OF and FD proved pathologically in 50 cases was conducted to discuss their imaging difference. Results: Sixteen of 24 cases with OF in medullary cavity of craniofacial bone and 6 cases with OF in the anterior cortical bone of tibia showed an expansive well-defined lesions with calcium-like high density. In 26 cases with FD, craniofacial OF showed ill-defined diffuse extension and "ground-glass" high density in medullary cavity. Cystic or "ground-glass" appearance were often seen in FD of extremity bone. Conclusion: The main imaging differential diagnosis between OF and FD in craniofacial bone is their growth pattern, margin and size. X-ray film combining with other imaging modalities is a main imaging examination procedure for them.
