脑瘫高危因素与出生胎龄的关系

目的探讨脑瘫高危因素与出生胎龄的关系及其可能的发病机理。方法回顾性分析478例脑瘫患儿的出生胎龄、高危因素与临床特征。结果宫内窘迫、先兆流产史、母孕期非感染性疾病、缺氧缺血性脑病、新生儿惊厥、新生儿感染、颅内出血及新生儿溶血与患儿出生胎龄无明显相关（P>0.05）；脑发育畸形、母孕期感染性疾病、吸入性肺炎、新生儿窒息、高胆红素血症与患儿出生胎龄负相关（P<0.05）。早产儿及足月儿痉挛型双瘫比率有显著性差异（P<0.05）。结论出生胎龄越小，合并脑发育畸形、母孕期感染性疾病、吸入性肺炎、新生儿窒息、高胆红素血症时，其脑性瘫痪患病率越高；早产儿患痉挛型双瘫比例大于足月儿。
