ⅠA期非小细胞肺癌冷冻消融疗效分析

目的 观察CT引导下氩氦刀冷冻消融治疗ⅠA期非小细胞肺癌的可行性、并发症及疗效.方法 经医院伦理委员会批准2008年6月至2009年11月接受氩氦刀冷冻治疗的ⅠA期非小细胞肺癌患者29例,其中男24例,女5例,平均年龄(62.1±7.4)岁,包括细支气管肺泡癌(BAC)3例,腺癌17例,鳞癌9例,经术后6、12、24个月随访观察,统计原位肿瘤复发、生长情况.结果 所有患者均未出现严重并发症;29例患者中,术后6个月复查时可见2例患者肿瘤组织灭活不完全,病灶保持相对稳定,其后复查中发现病灶处于进展状态;其余27例患者6个月内原位未见肿瘤组织生长,其后随访过程中发现3例患者出现纵隔淋巴结转移;3例患者出现远处转移;21例患者经术后2年随访,未见肿瘤复发或转移征象.结论 氩氦刀冷冻治疗能有效灭活肿瘤,且安全性高,值得临床推广.

Cryotherapy ablation for Stage I A non-small cell lung cancer

Objective To investigate the therapeutic effect and complications of CT-guided percutaneous cryoablation for Stage I A non small cell lung carcinoma（NSCLC）. Methods From June 2008 to November 2009, 29 cases of Stage I A NSCLC were treated by percutaneous cryoablation. All the patients were followed-up for 24 months after the operation, using contrast-enhanced CT to evaluate the tumor size and enhancement. Results There was no serious complications occurred;follow-up in the 6th month showed that 2 lesions show SD, others show CR. In 24 months, 3 patients were found lymph node of mediastinal enlarged. 3 patients were found metastasis,21 patients show CR. Conclusions CT-guided percutaneous cryoablation is a simple, safe and reliable procedure for NSCLC with the maximum diameter less than 3 cm.
