阴囊皮瓣在阴茎创面修复中的运用

目的探讨阴囊皮瓣在阴茎创面修复中的作用。方法采用阴囊任意皮瓣修复皮肤软组织缺损21例,清洁创面采取一期修复,污染创面彻底清创后修复,感染创面待感染控制、坏死组织完全脱落后二期修复,供瓣区均直接缝合。结果1例因排尿困难发生伤口裂开,其余20例皮瓣全部成活,切口一期愈合。术后随访6个月~6年,皮瓣修复的创面局部弹性良好,无1例发生勃起功能及排尿功能障碍,远期效果较好。结论阴囊皮肤血供丰富,伸展性好,阴囊任意皮瓣长:宽达3:1仍能较好存活。可修复阴茎任意部位创面,供瓣区一般能直接缝合,术后阴茎阴囊功能均未受较大影响,是简单易行、安全有效的阴茎创面修复的方法之一。

The use of scrotal flap in repairing penile wound

Objective To report the experience in repairing penile wound with scrotal flap.Methods Twenty-one cases of penile skin soft tissue defects were repaired with scrotal flaps.Clean wounds were performed primary repair.Polluted wounds were repaired after debridement.Infected wounds were repaired by second intention after infection was controlled and necrotic tissue completely ablated.The donor sites of flaps were sutured directly.Results One wound splited because of unexcreting urine,and the rest flaps were primarily survived.All patients were followed up from 6 months to 6 years.The color,texture and appearance of the flaps were good.Conclusion The scrotal skin has abundant blood supply and good extensibility.The donor sites of flaps can be sutured directly.It is an easy,safe and effective protocol in repairing penile wound.
