心房颤动抗凝治疗的临床分析及探讨

目的:探讨心房颤动患者抗凝治疗的重要性,以降低血栓栓塞事件的发生率.方法:对自1998年5月～2000年5月曾在我院治疗的69例房颤患者给予口服阿司匹林治疗.自2000年8月～2002年10月在我院住院治疗的63例房颤患者给予静脉注射肝素或皮下注射低分子肝素继之口服华法林治疗.对两组患者的病情转归随访1年的血栓栓塞事件发生率进行对比分析.结果:经用肝素低分子肝素和华法林抗凝治疗,INR达到国际标准2.5后,其血栓栓塞事件发生率较仅服阿司匹林非抗凝治疗明显下降.抗凝治疗者出现1例上消化道出血,无出血性死亡病例.结论:肝素低分子肝素及华法林对降低心房颤动所致血栓栓塞性事件发生率有效,是提高心房颤动患者的生存率和生存质量的有效治疗措施之一.

The Clinical Analysis of Anticoagulant Treatment for Patients Who Suffered from Atrial Fibrillation

Objective: To investigate the importance of anticoagulant treatment for patients who suffered from atrial fibrillation and reduce the incidence rate of thromboembolism case. Methods: 69 patients with atrial fibrillation were given to take asipirin, who come to our hospital from May 1998 to May 2000. Then sixty - three patients with the same case were asked to take warfarin after injection of heparin intrvenously and low molecule heparin subcutaneously from August 2000 to October 2002. We follow the end - point events at 1 year. Results: The rate of occurrence of thromboembolism is much lower in patients who take warfarin and hepain, with their international standard INR arriving 2.5 than in patients who only take aspirin. One patient with anticoagulant treatment suffered from upper gastointestinal hemorrhage. There are no case who died from bleeding. Conclusion: Heparin and Warfarin have a good effect on the curative effect for reducing the occurrence rate of thromboembolism which resulted from atrial fibrillation. It is one of the effective treatment strategy in rising the rate of survival and improving quality life for patients with atrial fibrillation.
