食管及贲门癌术后并发症再手术治疗（附17例分析）

目的：分析对食管及贲门癌术后几种常见并发症施再手术治疗的临床意义。方法：对１７例食管及贲让癌术后发生吻合口瘘、乳糜胸、胃穿孔等并发症的患者进行再次开胸分别施行吻合口重建，胸导管结扎及胃穿也修补术，并对再手术治疗的治愈率，死亡率进行统计分析。结果：经再手术治疗的１７例中因乳糜胸行胸导管结扎术７例，治愈４例，死亡３例。因吻合口瘘行吻理建术５例。治愈３例，死亡１例，１例自动出院；因胸内胃穿孔行胃穿孔修补

Re-operation for postoperative complications in esophageal and cardiac cancer treated with radical surgery

Objective:To analyze the clinical significance of surgical treatment for several common complications after radical esophageal and cardiac cancer surgery.Methods:Second thoracotomy was performed in 17 cases of esophageal and cardiac carcinoma complicated by anastomotic fistula,chylothorax,gastric perforation,ect;anastomotic reconstraction,ligation of thoracic duct and repair of gastric perforation were performed respectively.Results:Of the 17 cases receiving reoperation,7 with chylothorax had thoracic duct ligation,of which 4 cured and 3 dead;5with anastomotic fistula received reconstruction,of which 3 cured,1 dead and 1 withdrew from treatment,4 with perforated thoracic stomach received repair surgery,of which 3 cured and 1 withdrew from treatment,1 with necrosis of the transplanted colon after colon interposition received resection of the necrotic colon plus jejunum fistulization but died of severe infection.Conclusion:Second thoracotomy was an effective management for chylothorax,anastomotic fistula and thoracic gastric perforation presented after radical esophageal and cardiac cancer surgery and should be actively carried on when conservative treatment failed.
