阻塞性睡眠呼吸暂停低通气综合征患儿血清瘦素和肿瘤坏死因子-α水平的变化

目的探讨阻塞性睡眠呼吸暂停低通气综合征（OSAHS）患儿血清瘦素（LP）和肿瘤坏死因子-α（TNF-α）水平的变化及其与OSAHS的关系。方法采用放射免疫法检测68例OSAHS患儿及30例正常儿童血清LP和TNF—α水平，比较其组间差异，并分析OSAHS患儿血清LP和TNF—α水平与睡眠呼吸紊乱指数（AHI）和最低血氧饱和度（SαO1）的相关性；OSAHS组患儿进行持续气道正压通气（CPAP）治疗6个月后复查上述各项指标，比较治疗前后血清LP和TNF—α水平的变化。结果OSAHS组血清LP和TNF—α水平较对照组升高（P均〈0．05），OSAHS患儿血清LP和TNF-α水平与AHI呈正相关（r=0．451，0．460，P〈0．05），与其最低SαO2呈负相关（r=-0．489，-0.478，P〈0.05）；OSAHS患儿经CPAP治疗后血清LP和TNF—α水平较治疗前减低（P均〈0．05）。结论OSAHS患儿血清LP和TNF-α水平升高与其低氧血症有关；CPAP可改善OSAHS患儿缺氧状态，降低血清LP和TNF—α水平。

Research on the level of serum leptin and tumor necrosis factor-a in children with obstructive sleep apnea-hypopnea syndrome

Objective To explore the change of the serum leptin and tumor necrosis factor-α levels in children with obstructive sleep apnea-hypopnea syndrome and their relationship.Methods The level of serum leptin and TNF-α were determined in 68 children with obstructive sleep apnea-hypopnea syndrome and 30 healthy children by radio-immunity assay,and the results from patients treated with continuous positive airway pressure(CPAP)for six months were compared.The correlation between leptin and the levels of tumor necrosis factor-α,and apnea hypopnea index(AHI)or the minimum saturation of bloed oxygen were analyzed.Results The level of leptin and tumor necrosis factor-α in OSAHS patients were hisher than those in control(P＜0.05).The levels of leptin and tumor necrosis factor-αwere positive correlated with the AHI(r=0.451,r=0.460,P＜0.05)and negative correlated with the minimum saturation of blood oxygen(r=-0.489.r=-0.478,P＜0.05).The levels of leptin and tumor necrosis factor-α were decreased after treatment.Conclusion The levels of leptin and tumor necrosis factor-α are increased in OSAHS children.The increase of leptin and tumor necrosis factor-α is correlated with hypoxemia,and the levels of leptin and tumor necrosis factor-α can be improved by CPAP in OSAHS children.
