经脐腹腔镜胆囊切除术的临床应用

目的:探讨经脐腹腔镜胆囊切除术的可行性及技术难点。方法:回顾分析8例患者行经脐腹腔镜胆囊切除术的临床资料,采用单孔腹腔镜手术器械,于脐部做2～2.5cm切口。结果:8例手术均获成功。手术时间45～189min,平均70min。无胆漏、出血、切口感染等并发症发生;术后12h进流质饮食,第2天出院,3周后复查,脐部疤痕不明显。结论:经脐腹腔镜胆囊切除术安全可行;但其手术操作较传统LC困难,应注意手术适应证与器械的选择。其微创及美容效果明显优于传统LC,具有临床推广价值。

The clinical application of transumbilical laparoscopic cholecystectomy

Objective:To explore the feasibility and technical difficulties of transumbilical laparoscopic cholecystectomy(LC).Methods:Retrospective analysis was made on the clinical data of 8 patients who underwent transumbilical LC.One 2-2.5cm incision was made at umbilicus,and single-port laparoscopic instruments were used to complete the operative procedure.Results:All the eight operations were successful.Operative time was 45-189min,with an average of 70min.No bile leakage,bleeding,wound infection or other complications occurred.Patients could eat liquid diet 12h after the operation,and were discharged the next day.3 weeks later,umbilical scar was not obvious.Conclusions:The transumbilical LC is safe and feasible,but its operative procedure is more difficult than the traditional LC,surgeon should pay attention to the choice of surgical indications and instruments.In respect of minimally invasive and cosmetic results,the transumbilical LC is much better than conventional LC,and has great value of clinical practice.
