依替米星和异帕米星致耳毒性的病例分析

目的:探讨依替米星和异帕米星致耳毒性的类型和规律,为临床安全用药提供参考.方法:对2005年11月-2010年11月北京市药品不良反应监测网中关于依替米星和异帕米星耳毒性的病例报告进行回顾与分析.结果:应用依替米星和异帕米星后发生耳毒性的病例分别为27例和7例,其中患者年龄分别有12倒(44.4％)和3例(42.9％)大于60岁；耳毒性主要临床表现为耳鸣、听力下降；依替米星和异帕米星分别有20例( 74.1％)和4例(57.1％)患者的耳毒性发生于用药后0～72h；应用依替米星和异帕米星发生耳毒性后及时停药,好转的分别为18例(66.7％)和5例(71.4％).结论:依替米星和异帕米星可引起耳鸣、听力下降等耳毒性表现,老年患者所占比例较高；症状多在用药后0～72h内发生,停药后耳毒性症状多可消失或好转.

Cases of ototoxicity caused by etimicin and isepamicin

Objective： To investigate the types and patterns of ototoxicity caused by etimicin and isepamicin, and to provide evidence for safe use of the drug. Methods： The case reports of ototoxicity caused by etimicin and isepamicin were reviewed and analyzed from Adverse Drug Reaction Monitoring Network of Beijing from November 2005 to November 2010. Results： There were 27 and 7 cases of ototoxicity related to etimicin and isepamicin, respectively. Among them, 12 （44.4%） and 3 （42.9%） cases were more than 60 years old, most of which were tinnitus and hearing loss. Ototoxicity occurred at 0 - 72 h after treatment in 20 （74.1%） cases of etimicin and 4 （57.1%） cases of iseparnicin. Ototoxicity relieved in 18 （66.7%） cases of etimicin and 5 （71.4%） cases of isepamicin after withdrawal. Conclusion： Etimicin and isepamicin can cause tinnitus, hearing loss and other symptoms of ototoxicity. The proportion of elderly patients was high. Most symptoms occurred within 0 - 72 h after dosing and disappeared or relieved after etimicin and isepamicin withdrawal.
