冠状动脉侧支循环在心肌梗塞中的临床价值研究

目的：探讨冠状动脉侧支循环的临床价值。方法：对６８例心肌梗塞（ＭＩ）患者进行冠状造影及三年随访资料研究，其中３６例侧支积分〉１（ＣＣ组），另３２例侧支积分〈１＊（ＮＣＣ组）。结果：与ＮＣＣ组比较，ＣＣ组的多支病变及冠脉次全闭塞率较高（Ｐ〈０．０１），侧支循环及心功能较好（Ｐ〈０．０１～０．０５）。两组的冠脉狭窄，迂曲，钙化程度相似（Ｐ〉０．０５）。随访研究发现，ＣＣ组的近两年优于ＮＣＣ组（ＪＫＰ〈

The clinic value of coronary artery collatcral circulation on myocardioinfarction

s: Objectives:To study the clinic value of coronary artery collateral circulation.Methods:Among 68 patients with myocardioinfarction(MI), there are 36 cases with collateral circulation integration>1(CC group),32 with collateral circulation integration<1(NCC group).Coronary angiography and follow up study were performed in all the patients.Results:Compared with NCC group by coronary angiography,the CC group had higher multiple arteries damage rate and high coronary subtotal angiolysis rate (P<0.01),more collateral circulation (P<0.01) and better heart function (P<0.05).However,the coronary stenosis tortuosity and calcification rate of the two groups were similar (P>0.05).The follow up study found that,in two years ,the CC group has a better heart function (P<0.01),higher angina pectoris and re infarction rate (P<0.01) lower mortality (P<0.01).But the groups had similar prognosis in the third year.Conclusion:The existence of collateral circulation is one of the reasons of postinfarction angina pectoris and reinfarction;CC has important protective effect in acute phase and in two years,but no protective effect for the third year.
