胰腺结石的超声内镜检查和ERCP的对比研究

目的 探讨胰腺结石的EUS和ERCP表现及其诊断价值。方法 分析经手术或内镜下胰管取石确诊35例胰腺结石患的EUS和ERCP检查结果。结果 35例中ERCP诊断27例(78．3％)，8例诊断不明。伴有胰实质结石的4例患ERCP均未能对胰实质结石做出诊断。而EUS诊断33例(94．3％)，其中4例胰实质结石全部显示，2例胰管小结石未能诊断，EUS和ERCP联合检查全部35例均获得诊断。结论 ERCP联合EUS可提高胰腺结石诊断的准确率。

The value of EUS and ERCP in diagnosis of pancreatic stones.

Objective To discuss EUS and ERCPO presentations of pancreatic stones and evaluate the diagnostic value of EUS and ERCP. Methods EUS and ERCP findings of 35 patients with pancreatic stones confirmed by surgery or endoscopy from January 1998 to February 2003 in this hospital were analyzed. Results ERCP detected 27 cases (78. 3%) of pancreatic stones, and was not able to confirm the remaining 8 cases and failed in the diagnosis of 4 cases of accompanying stones of pancreatic parenchyma. EUS detected 33 (94. 36%) cases of pancreatic stones and 4 cases accompanying stones of pancreatic parenchyma, and failed in the diagnosis of the remaining 2 cases where the stones were small. Combined use of ERCP and EUS detected all 35 cases of pancreatic stones. Conclusions Combined use of ERCP and EUS can greatly improve the diagnostic accuracy of pancreatic stones.
