AF钉间接减压复位治疗胸腰椎重度爆裂骨折

目的: 探讨AF钉间接减压治疗重度胸腰椎爆裂骨折的临床效果。方法: 50例胸腰椎爆裂骨折患者用间接减压复位内固定植骨术。结果: 随访6个月~9年, 平均3. 5年。伤椎高度由术前的40%恢复到术后的95%, 椎管狭窄指数术前平均2, 术后平均<1。术后除12例全瘫病例无神经功能恢复外, 不完全截瘫患者, 术后神经功能按Frankel分级, 均有Ⅰ级以上恢复。结论: AF钉系统具有操作简单、固定可靠, 能有效的间接减压, 并能很好的恢复脊柱的解剖序列, 是一种治疗重度胸腰椎爆裂骨折的首选方法。

Indirect decompression and reduction treating severely burst thoracolumbar fractures with AF nail

Objective:To explore and evaluate the clinical result of indirect decompression and reduction treating severely burst thoracolumbar fractures with AF nail.Method:The author treated 50 cases of severely burst thoracolumbar fractures with indirect decompression and reduction plus inner fixation and bone plant.Result:The average follow-up time was 3.5 years (6 months to 9 years).The height of burstvertebrae were 40% of normal preoperatively and 95% of normal postoperatively.The average index of spinal canal stenosis was 2 preoperatively and less than 1 postoperatively.Despite the 12 cases of complete paralysis all the other incomplete paralysis cases had a recovery of at least one grade according to Frankel classification.Conclusion:The system of AF nail has advantages of easy operating,firm fixation, indirect decompression capability and spines can recover to normal shape.This technique is an optimal method for treating severely burst thoracolumbar fractures.
