标本双份测定法在临床生化检验室内质控中的应用

目的利用极差型质控图,探讨患者标本双份测定法在临床生化检验室内质控中的应用。方法在常规室内质控在控后开始检测标本,每天从前10份标本中选出1份总蛋白(TP)60～80 g·L~(-1)、白蛋白(ALB)30～50 g·L~(-1)的标本,3 h后再重测1次该标本的TP、ALB,每天计算2次测定结果的差值和差值的标准差,连续40个工作日,利用标准差计算双份测定极差的控制限,建立极差型质控图。再连续1个月(22个工作日)进行患者标本TP、ALB双份测定,把每天2次测得结果的差值描到极差型质控图上。结果TP和ALB 1个月标本双份测定差值无失控点。结论患者标本双份测定法可以用来监测检测系统的稳定性,与常规室内质控相互补充提高检验质量。

Application of the duplicate determination of patient samples in clinical biochemical internal quality control

Objective To explore the application of the duplicate determination of patient samples in clinical biochemical internal quality control by using the extreme difference quality control chart.Methods In the conventional internal quality control in the control specimens after the test day,a specimen which total protein(TP) result was 60-80 g·L~(-1),and albumin (ALB) result was 30 -50 g·L~(-1) was selected from the first ten specimens every day,the specimen's TP and ALB results were retested after 3 hours.The difference between the two test results and the difference of the standard deviation were calculated every day for 40 days.Extreme difference quality control chart was established by control limits of range which was calculated by the standard deviation.And then TP and ALB of specimens was test with duplicate determination for a month(22 working days), the difference between the two test result was described on the extreme difference quality control chart.Results The difference of the results of TP and ALB with duplicate determination was no loss of control point in a month.Conclusion Duplicate determination of patient samples can be used to monitor the system stability,and it can improve the inspection quality complementary to the method of conventional internal quality control tests.
