超声及125CA在卵巢恶性肿瘤中的诊断价值

目的评价超声及血清125CA对于卵巢恶性肿瘤的诊断价值。方法分析经二维超声、彩色多普勒超声及血清125CA检查并经手术病理证实的卵巢癌患者109例。结果54例卵巢恶性肿瘤中,表现为囊性改变占9.5%;表现为囊实混合性为占64.6%;表现为实性改变占17.4%。超声检查及125CA在诊断卵巢恶性肿瘤上分别获得了90.8%、75.9%敏感度,96.4%、65.6%特异度,96.1%、68.3%阳性预测值,91.3%、73.5%阴性预测值,93.6%、71.6%准确度。超声检查比125CA具有更高的诊断价值,两者差异有统计学意义。联合使用可以提高敏感度和阴性预测值。结论超声诊断卵巢恶性肿瘤比125CA具有更高诊断价值;两者联合应用可以降低漏诊率,是筛查卵巢恶性肿瘤的良好办法。

Ultrasonographic Diagnosis of Ovarian Carcinoma :Comparision with Serum 125CA

Objective To evaluate the value of ultrasonography and serum carcinoma marker (~ 125 CA) in the diagnosis of ovarian carcinoma.Methods Ultrasound imaging,Color Doppler and serum~ 125 CA in 109 cases with ovarian carcinoma were analyzed.All cases were confirmed by pathology.Results Among 54 ovarian carcinoma in 109 patients,9.5% cystic tumors;64.6% cystic-solid tumors;17.4% solid tumors.The sensibility,specificity,accuracy,positive predictive value and negative predictive value of US were 90.8%,96.4%,93.6%,96.1% and 91.3% respectively,vs~ 125 CA 75.9%,65.6%,71.6%,68.3% and 73.5%,respectively,P<0.05.Combination of the two methods can improve the sensibility and negative predictive value.Conclusions US has higher sensibility,specificity,accuracy,positive and negative predictive values than those of ~ 125 CA.
