儿童先天性马蹄内翻足手术治疗中远期疗效评价

目的探讨手术治疗先天性马蹄内翻足 (CCF)的中远期疗效以及影响疗效的可能性因素.方法采用 Mckay 手术,术时以克氏针固定功能位,术后石膏固定6～8周.结果 430 例618足, 随访4～14 年,平均 8.6年,根据术后足的外观形态、足印、踝部功能、有无疼痛及 X 线检查进行评价:优318例 458 足;良80例113足;一般 32 例 47足;其中有平足14 例,足外翻2例,优良率92.57%.结论手术是矫正先天性马蹄内翻足畸形的主要方法,手术疗效与手术年龄及足畸形程度有关,非手术治疗可提高手术的治愈率 ,Mckay手术是治疗CCF 的一种较好的方法.

Medium and long term effect of mcKay operation in the correction of congenital clubfoot for children

Objectives To evaluate of medium and long term effect from McKay operation in the correction of congenital clubfoot (CCF) and other factors which may effect the curative results.Methods McKay operation with Kirschner fix the functional point and plaster fixed for 6-8 weeks.Results Four hundred and thirty patients (618 feet) were followed up postoperatively for 4 to 14 years, averaging8.6 years. According to the criteria concerning the external appearance, functional recovery of the foot after operation and roentgenographic findings, the result was evaluated as excellent in 318 patients (458 feet), good in 80 patients (113 feet), fair in 32 patients (47 feet). In which there are 14 patients with flatfoot deformity, 2 patients with valgus, the rate for excellent and good cases reaching 92.57%.Conclusion Operation is the main way for correcting the rigid CCF. The effect of operation is related to the age when the operation is given and the deformation degree of the feet. Non-operational cure can also increase the curative rate. It is considered that Mckay operation is quite effective for treating CCF.
