立体定向术治疗多发性脑脓肿(附11例临床分析)

目的通过对立体定向术治疗多发性脑脓肿的回顾性分析,总结手术经验。方法应用Leksell-G型定向仪,行MRI引导下立体定向穿刺 术中抗生素冲洗术,设计最佳手术入路,行多发脓肿穿刺,术后常规应用抗生素治疗2周。结果11例多发脑脓肿患者,共30个脓肿,均一次穿刺成功,经抽吸冲洗后复查脓腔消失。抗生素治疗2周后,经随访全部病例治愈。结论立体定向术具有定位准确、安全、创伤小等优点,适用于多发性脑脓肿及脓肿部位较深或位于语言、运动中枢等主要功能部位,或由于年老体弱或其他严重疾病不能耐受开颅手术者。

Stereotaxic surgery in the treatment of multiple brain abscesses(11 cases clinic study)

Objective A retrospective review of 11 patients with multiple brain abscesses who underwent stereotactic surgery.Methods 11 cases were operated by Leksell-G stereotactive apparatus.According to result of MRI enhancement scan,we accounted the coordinate of brain abscesses,punctured the cavity of abscesses.After scouring completely,we injected antibiotic in absesses.All patients were accepted the antibiotics to cure 2 weeks after stereotaxy surgery.Results Total 11 patients had 30 brain abscesses.Operations were successed.Brain abscesses were missed and clinic symptom were alleviated.Thought the antibiotics cure 2 weeks,we visit all cases to cure.Conclusion It is an effective and safe method that stereotaxy surgery treat multiple brain abscesses.It is applicable to multiple brain abscesses,deep brain abscesses,in main function part of language and sport,the patients who could not bear neurosurdery surgery.
