摘    要:目的:介绍外踝上逆行岛状筋膜皮瓣修复足部严重电烧伤、热压伤的疗效。方法:自2001年以来,我科共利用外踝上逆行岛状筋膜皮瓣修复踝部电烧伤5例,足背热压伤4例,足跟部慢性溃疡3例。结果:12例皮瓣,10例全部成活,2例皮瓣因蒂部张力过大而致其远端部分坏死。术后随访6个月~4年,外形满意,功能良好,均重返工作岗位。结论:该术式具有操作简单,供瓣面积大,血运可靠,且不牺牲小腿主要动脉。该皮瓣对于小腿中下1/3、足跟、内、外踝及足背等较大面积缺损的修复,是一种较理想的方法,值得临床推广应用。

Abstract:Objective: To introduce outcome of treating foot deep burn of electrical injury and crush thermal injury using reverse lateral supramalleolar artery island fascio - cutaneous flap. Methods: A reverse island fasciocutaneous flap was applied to repair 5 cases of the electrical burn injury and 4 cases of crush thermal injury at the dorsum of the foot and 3 cases of the calcaneal region ulcers. Results: 10cases of 12 flap grafting were survived, 2cases were partial necrosis in the rarest end because of pedicel tension being justo major. During postoperation follow up of a period of 6 months - 4 years, satisfactory results in function and cosmetic point were obtained. All returned their positions of the job. Conclusions: The operation is a simple, easy to operate way. It has such an advantage as a large flap available without sacrifice of a main blood vessel. It is an excellent method for repair of the deep burn in the lower 1/3 part of leg, in the heel, inner and lateral malleolus, dorsum pedis. It is valuable to populize.
