超声引导下浅表淋巴结穿刺活检在临床诊断中的应用价值

目的探讨超声引导下穿刺活检技术对浅表淋巴结组织疾病病理诊断中的应用价值。方法回顾性分析我科接受超声引导下浅表淋巴结穿刺活检的144例患者的临床资料。穿刺靶淋巴结大小0.6cm×0.4cm~6.1cm×3.6cm。选用18 G、16 G穿刺针活检。结果使用18 G穿刺针活检者108例,使用16 G穿刺针活检者36例。每次穿刺组织条数2~6条,组织条大小0.5cm×0.1 cm×0.1 cm~2.0cm×0.1cm×0.1 cm。138例患者所取组织量可以满足组织病理学诊断,取材满意率为95.8%(138/144)。穿刺活检组织病理学结果:淋巴结良性病变41例;淋巴结恶性病变97例;取材不佳6例。严重并发症发生率为零。结论超声引导下浅表淋巴结穿刺活检技术具有实时、简便、有效、并发症少等优点,对临床上淋巴结的组织病理学诊断有很好的应用价值。

Application value of ultrasound-guided superficial lymph node biopsy in clinical diagnosis

Objective To investigate clinical application value of ultrasound-guided superficial lymph node biopsy in histopathological diagnosis. Methods Clinical data of 144 patients who underwent ultrasound-guided superficial lymph node biopsy were analyzed retrospectively. The size of target lymph nodes ranged from 0.6 cm×0.4 cm to 6.1 cm×3.6 cm. 18 G and 16 G puncture needles were selected. Results 18 G puncture needle was used in 108 cases,and 16 G puncture needle was used in 36 cases. 2 to 6 tissues were punctured each time and the size of puncture tissues ranged from 0.5 cm×0.1 cm×0.1 cm to 2.0 cm×0.1 cm×0.1 cm. The amount of sample tissues acquired from 138 cases could meet the demand of histopathological diagnosis and the satisfaction rate of sampling was 95.8%（138/144）. The histopathological results of biopsy showed that 41 cases were benign lesions,97 cases were malignant lesions and 6 cases were unsuitable tissues. There were no severe complications. Conclusion The technology of ultrasound-guided superficial lymph node biopsy is real-time,convenient and effective with few complications. It is quite valuable for the clinical histopathological diagnosis on lymph nodes.
