经尿道前列腺电汽化术中并发症及防治(附1360例报告)

目的探讨经尿道前列腺电汽化术（TVP）术中并发症及防治对策。方法对1360例前列腺增生症患者行经尿道前列腺电汽化术。结果术中并发症包括：出血，其中需要输血者51例（占3．75％）；尿道外口损伤16例（占1．18％）；膀胱穿孔3例（占0．22％）；膀胱颈部分离15例（占1．1％）；前列腺包膜穿孔46例（占3．38％）；TURS12例（占0．88％），其中8例合并前列腺包膜穿孔；寒战44例（占3．24％）；冲洗液外渗21例（占1．54％）。结论TVP术中仍有一些并发症发生，但严格止血、正确操作、及早预防、操作视野清晰等可减少术中并发症的发生。

Intraoperative complications of transurethral electrovaporization of prostate (report of 1 360 cases)

[Objective] To review the intraoperative complications and preventative strategies of them in transurethral electrovaporization of the prostate (TVP). [Methods] 1 360 patients with symptomatic benign prostatic hyperplasia (BPH) were treated by transurethral electrovaporization of the prostate (TVP). [Results] The common intraoperative complications included: intraoperative bleeding, among which blood transfusion was required in 51 cases(3.75%), injury of urethral meatus in 16 cases(1.18%), perforation of the bladder in 3 cases(0.22%), abruption of bladder neck in 15 cases (1.1%), perforation of the prostatic capsule in 46 cases (3.38%), transurethral resection syndrome(TURS) in 12 cases (0.88%), in which 8 cases were accompanied by perforation of the prostatic capsule, chill in 44 cases (3.24%), exosmosis of the irrigation fluid in 21 cases (1.54%). [Conclusions] Although there are some intraoperative complications in TVP, carefully hemostasis, correctly procedure, early prevention, clear operative field are the key points to reduce the occurance of the intraoperative complication.
