早期新生儿肺炎诊治探讨

目的为了提高对早期新生儿肺炎的治愈率,探讨其早期诊断与治疗。方法对我院2000年9月~2005年3月收治的148例早期新生儿肺炎的临床表现及诊治情况进行回顾性分析。结果总结出早期新生儿肺炎临床特点(1)发病早,产前、产时感染多,(2)临床表现不典型,(3)多为早产儿和低体重儿,(4)重症肺炎并发症多,(5)痰培养多为革兰氏阴性杆菌,对第三代头孢菌素及阿奇霉素敏感性相对较高。结论早期新生儿肺炎临床表现更不典型,以革兰氏阴性杆菌感染为主,第三代头孢菌素疗效较好。重症肺炎多见,机械通气可减少病死率。

The study of diagnosis and treatment of 148 pneumonia in early newborn

Objective To explore early diagnosis and treatment of patients with pneumonia in early newborn, in order to improve the treatment of this disease.Methods Records of clinical manifestation, diagnosis and treatment of 148 cases of pneumonia in early newborn were analyzed retrospectively in our hospital since September 2000 to March 2005.Results The characteristices of the disease are: (1) Symptoms take place early and most patients with pneumonia in early newborn were caused by antepartum infections. (2)The patients have no typical clinical manifestation .(3)Most patients were prematures and low birth weight infants. (4) The complications were common many in serious cases. (5) Gram-negative bacteria were main organisms in sputum culture and most of these bacteria were susceptible to third generate cephalosporin.Conclusion Clinical manifestation of pneumonia in early newborn is not typical. Most of the patients are infected by gram-negative bacterium . The third generate cephalosporin is often effective in treatment .The pneumonia is often serious. Mortality of the patients can be decreased by mechanical ventilation.
