血管紧张素Ⅱ一型受体拮抗剂对大鼠系膜增生性肾炎干预效果的实验研究

摘    要:目的 观察血管紧张素一型受体拮抗剂缬沙坦对大鼠系膜增生性肾炎病理及相关临床指标的干预作用。方法 采用大鼠Thy-1肾炎模型,将肾炎大鼠分为Thy-1肾炎组及缬沙坦治疗组。检测两组疾病诱导后第1、3、5、7天平均动脉血压、24h尿蛋白排泄量、血浆肌酐及尿素氮,并取肾脏行光镜及电镜检查,免疫组化检测肾小球内PCNA蛋白表达。同时设正常对照。结果 同肾炎组比较,缬沙坦治疗组第3-7天24h尿蛋白泄量减少(P<0.05),肾小球有核细胞增多程度下降(P<0.05),系膜细胞增生减少,系膜区扩张也减少(P<0.05),肾小球内PCNA阳性核减少(P<0.05)。平均动脉血压、血浆肌酐及尿素氮较正常对照无显著差异。结论 对大鼠系膜增殖性肾炎,缬沙坦能一定程度抑制系膜增生,减轻肾小球病理损伤,降低蛋白尿。

Intervention of Angiotensin II receptor antagonist valsartan on mesangial proliferative glomerulonephritis in rats

Abstract:Objective To evaluate the intervention effects of valsartan, specific antagonist of angiotensin II receptors on the kidney of rat with Thy1 glomerulonephritis. Methods The Thy1 glomerulonephritic rats were treated with valsartan. Mean arterial blood pressure, 24 hour urine protein excretion, serum creatinine and blood urea nitrogen as well as renal morphological lesions were observed day 1, 3, 5, and 7 of Thy1 nephritis, meanwhile PCNA expression in glomerulus was measured with immuohistochemistry. A normal control group and untreated Thy1 nephritic group were also studied. Results In valsartan treated nephritic rats, the proteinuria,glomerular hypercellularity, mesangial matrix expansion and PCNA expression were significantly reduced when compared with the untreated rats from day 3 to 7 of Thy1 nephritis( P <0.05). While no change was noted in mean arterial pressure, serum creatinine and blood urea nitrogen in all rats. Conclusion Valsartan partly reduces the urine protein excretion and inhibits the proliferation of mesangial cells in normotensive rats with Thy1 nephritis, and therefore ameliorates the glomerular morphological lesions in nephritic rats.
