恶性肿瘤相关物质(TSGF)在诊断小儿恶性肿瘤中的应用

目的探讨恶性肿瘤相关物质(Tumorsuppliedgroupoffactors,TSGF)在小儿恶性实体瘤诊断及预后判定中的临床意义。方法对恶性肿瘤组(98例)、良性肿瘤组(85例)、正常组(268例)及感染组(31例)血清中TSGF的浓度分别进行检测,并作动态观察。结果各种常见的小儿恶性肿瘤血清中TSGF浓度与正常组比较明显升高(P<0.01),良性肿瘤组与正常组比较无差异性。以60U/ml为正常参考值,则诊断灵敏度89%、特异性为87%。感染组患儿在治疗前血清中TSGF浓度与正常组比较也有升高(P<0.01),但治疗后2周血清中TSGF浓度会下降至正常范围内。恶性肿瘤经有效治疗后,TSGF有下降的趋势,可降至正常范围内;而病情恶化及肿瘤复发者,血清TSGF有升高的现象。结论TSGF对小儿恶性实体瘤有诊断意义,具有很高的特异性、敏感性和广谱性。炎症对TSGF有很强的干扰性,当使用TSGF作恶性肿瘤诊断时,应排除炎症因素。

The role of tumor supplied group of factors (TSGF) in diagnosis of malignant massive tumor in children

Objective To investigate the role of tumor supplied group of factors (TSGF) in diagnosis of malignant massive tumors in children.Methods The levels of TSGF in serum were measured in 268 children of normal controls, 85 patients with benign tumors, 31 patients with acute infection and 98 patients with malignant massive tumors. Moreover, levels both in malignant tumors and infection groups were analyzed dynamically before and after treatment.Results The levels of TSGF in serum of malignant tumor were significantly higher than those of the normal controls ( P < 0.01 ). After treatment, the levels of TSGF in malignant group were decreased. There was no significant difference between the benign tumors and those of the normal group ( P > 0.01 ). The sensitivity and specificity of TSGF in diagnosis of malignant massive tumors in children were 89% and 87%. The levels in the group of patients with infection before treatment were higher than those in the normal controls ( P < 0.01 ), but they came down to the normal range after treatment ( P > 0.01 ).Conclusions Measurement of TSGF in serum might be used in the diagnosis of malignant massive tumor in children if the acute infection can be excluded.
