肝癌三维适形放射治疗29例分析

目的评价肝癌三维适形放射治疗(3DCRT)的临床疗效。方法对29例不能手术治疗的肝癌实行大分割、高剂量三维适形放射治疗,总剂量DT50Gy,分10次,22天内完成。结果治疗结束后6个月随访,CT或MR I检查证实:肿瘤完全缓解20.7%(6/29),部分缓解51.7%(15/29),稳定17.3%(5/29),进展6.8%(2/29)。甲胎蛋白(AFP)不同程度下降。1年、2年生存率分别为48.3%(14/29)和17.2%(5/29)。结论三维适形放射治疗为不能手术切除肝癌的治疗提供了一个疗程短、局部控制率高、毒副作用小的好途径。

Three- dimensional Conformal Radiation Therapy for Non- surgical Hepatic Carcinoma

Objective To evaluate the effects of 3-dimensional conformal radiation therapy(3DCRT) for liver cancer.Methods 29 patients with non-surgical liver cancer treated with high fractionation,high dose 3-dimensional conformal radiation therapy(3DCRT),to the total dosage of 50Gy/10Fx/22D.The tumors were irradiated discontinuously,in total 10 times,5.0Gy each time.Results The clinical symptoms of the patients were improved when radiation was finished.Follow-up was performed six months later after treatment.Out of the 29 patients,complete response was 20.7%,partial response was 51.7%,no change was 17.3% and progressive disease was 6.8%.AFP was descended.The 1,2-year survival rate of this group was 48.3% and 17.2%.Conclusion This is an effective way of shorter course,higher local control rate,slighter side effects for non-surgical liver carcinoma treatment with 3-dimensional conformal radiation therapy.
