胃癌中D2-40标记的淋巴管密度的检测及表达意义

目的探讨胃癌组织中微淋巴管分布、密度、形态特征及其与相关临床病理因素之间的关系,分析胃癌淋巴结转移的可能机制.方法应用单克隆抗体D2-40检测49例原发性胃腺癌淋巴管情况,并分析其和临床病理参数之间及和癌周淋巴结转移之间的关系.结果人胃癌癌旁区淋巴管密度高于肿瘤区,肿瘤癌旁区域中淋巴管密度与肿瘤的浸润深度、分化程度、淋巴结转移,差异有统计学意义（P〈0.05）.但在年龄、性别上无差异性.结论 （1）胃癌组织中淋巴管的分布及形态存在异质性,淋巴管主要分布于肿瘤边缘区,肿瘤边缘区淋巴管密度与淋巴结转移、分化程度、浸润深度有关,其淋巴管的数量的增加和形态的变化可能与胃癌的侵袭及转移相关;（2）胃癌癌旁组织中的淋巴管密度检测可作为评估胃癌淋巴结转移状况的一项辅助指标,为判断胃癌患者预后及确定临床治疗方案提供实验和理论依据.

Detection of D2-40 Marked Lymphatic Vessel Density in Gastric Cancer and Its Clinical Signincance

Objective To investigate the characteristics of distribution,density,and morphology of lymphatic vessel in gastric cancer and the correlation with its clinicopathological factors,and analyse the possible mechanism of lymph node metastasis in gastric cancer.Methods Monoclonal antibody D2-40 was used to detect the lymphatic invasion in 69 cases of primary gastric adenocarcinomas,and the relationships between lymphatic invasion and clinicopathological findings or lymph node micrometasis were analyzed.Results Compared with the region of human gastric cancer the tumor borderline was found with a significantly high lymphatic vessel density（LVD）.The LVD in the borderline had positive correlation to the depth of tumor invasion,lymph node metastasis and the diferenfiation degree（P0.01）but there was no significant difference in age and sex.Conclusions（1）The lymphatic vesseles with heterogeneity appear in the gastric cancer,and mainly distribute in the tumor borderline.The LVD in the tumor borderline has correlation with lymph node metastasis,diferenfiaton degree,and the depth of tumor invasion.The increase of LVD may be related to the invasion and metastasis of gastric cancer in the tumor borderline.（2）The detection of LVD in the adjacent tissue of gastric cancer can be a important affiliated marker to evaluate the lymph node metastasis.It also can measure the prognosis of gastric cancer and provide experimental and theoretical foundation to confirm the clinical treatment methods.
