降钙素原在新生儿细菌感染诊断中的应用价值

摘    要:目的 探讨血清降钙素原(PCT)在新生儿细菌感染诊断中的应用价值。 方法 选取110例感染患者为实验组,82例同期非感染患者为对照组,对比分析两组患者的PCT和C反应蛋白(CRP)水平。 结果 重症感染组和局部感染组的PCT、CRP水平和PCT、CRP阳性率高于对照组,重症感染组的PCT、CRP水平和PCT、CRP阳性率高于局部感染组,差异有统计学意义(P<0.05)。革兰阳性菌组与革兰阴性菌组PCT、CRP水平差异无统计学意义(P>0.05)。PCT细菌感染诊断的灵敏度为89.09%,特异度为91.46%。CRP细菌感染诊断的灵敏度为81.82%,特异度为90.24%。治疗有效组PCT、CRP水平随时间而降低,治疗无效组PCT水平随时间而升高,CRP水平的变化则较不明显。 结论 PCT检测可以用于细菌感染的早期诊断,并对重症感染敏感性更高,可用于患者的病情监测和用药指导。

Application value of procalcitonin in the diagnosis of neonatal bacterial infection

Abstract:Objective To explore the application value of procalcitonin in the diagnosis of bacterial infection in newborns. Methods One hundred and ten infected newborns were served as the experiment group, and 82 non-infected newborns were simultaneously considered as the control group. The levels of procalcitionin (PCT) and C-reactive protein (CRP) were compared between the two groups. Results The levels and positive rates of PCT and CRP in the severe infection group and the local infection group were all higher than those of the control group, and the levels and positive rates of PCT and CRP were higher in the severe infection group than in the local infection group, with statistically significant difference (P<0.05). No statistically significant difference was found in the levels of PCT and CRP between the Gram-positive bacteria group and the Gram-negative bacteria group (P>0.05). The sensitivity and specificity of PCT in the diagnosis of bacterial infection were 89.09% and 91.46%, respectively. The sensitivity and specificity of CRP in the diagnosis of bacterial infection were 81.82% and 90.24%, respectively. The levels of PCT and CRP of the effective treatment group decreased with the time, but the level of PCT of the ineffective treatment group increased with the time. No obvious change was found in the level of CRP. Conclusions Determination of PCT can be used for early diagnosis of bacterial infection, with high sensitivity for detection of severe infection. It is suitable for disease monitoring and medication guide in bacterial infected patients.
