辅助孵化对高龄妇女IVF-ET临床结局的影响

摘    要:目的探讨辅助孵化对高龄妇女体外受精-胚胎移植临床结局的影响。方法回顾性分析96例高龄妇女体外受精-胚胎移植周期,其中行激光辅助孵化后移植52周期(AH组),常规胚胎移植44周期(常规组),比较两组患者临床妊娠率和胚胎种植率。结果AH组的临床妊娠率为28.85%(15/52),胚胎种植率为18.10%(19/105);常规组的临床妊娠率为20.45%(9/44),胚胎种植率为8.93%(10/112)。当基础卵泡刺激素(FSH)≥10(IU/L)或透明带厚度≥18?μm时,AH组胚胎种植率明显高于常规组(P<0.05)。结论对于高龄妇女,尤其对于合并基础FSH≥10(IU/L)或移植胚胎透明带厚度≥18?μm的患者在胚胎移植前行辅助孵化有助于提高胚胎种植率。

Abstract:Objective To study the effect of assisted hatching on in vitro fertilization-embryo transfer (IVF-ET) in women with advanced age. MethodA retrospective analysis of 96 IVF-ET cycles was used to compare embryo transfer after laser-AH (n=52) and traditional CT-ET(n=44). Results18.10% (n=19) of the 105 embryos exposed to AH were implanted, and 8.93% (n=10) of the 112 embryos not exposed to AH were implanted (P<0.05, (2 test). There were no differences in the rate of clinical pregnancy between the two groups (28.85% VS 20.45%, P>0.05). When basal FSH≥10(IU/L) or embryos for transfer with zona thickness ≥18μm, laser-assisted hatching increased the implantation rate (P<0.05, (2 test). Conclusion Assisted hatching increases the implantation rate of IVF-ET in women with advanced age, especially in those with the basal FSH≥10(IU/L)or embryos for transfer with zona thickness ≥18μm.
