超声引导及神经刺激仪在臂丛神经阻滞中的应用

目的：探讨临床中更安全有效的臂丛神经阻滞的辅助手段。方法：将90例拟行上肢手术的病人随机分成3组,超声引导组（U组）、神经刺激仪组（S组）,传统方法对照组（C组）,每组30例。3组均给予0.375%的罗哌卡因20 ml进行臂丛神经阻滞。结果：3组的阻滞成功率分别为U组96%,S组94%,C组80%。完全起效时间及麻醉效果3组相比,超声引导组和神经刺激仪组优于传统臂丛阻滞组。结论：采用超声引导穿刺针的穿刺及观测局麻药的扩散可以提高臂丛神经阻滞的成功率。

Application of Ultrsaound-guided and nerve Stimulator in Brachial Plexus Block

Objective：To observe the safety and efficacy of ultrasound- guided and nerve stimulation guided interscalene brachial plexus block in upper limb operation. Methyls： 90 patients undergoing upper limb operation with interscalene tracheal plexus block were divided into three groups （ n = 30 in each group）. Interscalene tracheal plexus blocks operated in group U were guided by the ultrasound, while in Group S the needle placement was guided by the nerve stimulation. The anesthetics were 0. 375 % ropivacaine 20 ml. Results： The onset time of block was significantly shorter in group U and group S than that of in group C. The fineness rate was higher than that of in group C. The complication or side - effects in all groups were not significantly difference. Conclusion： The safety and efficacy can be improved significantly when interscalene brachial plexus block is performed under ultrasound guidance and nerve stimulation.
