中低位直肠癌切除术中单用吻合器吻合结直肠的临床效果观察

目的探讨单用吻合器在中低位直肠癌切除吻合的临床效果。方法比较单用吻合器中低位直肠癌切除吻合162例和吻合器、闭合器双器具中低位直肠癌切除吻合158例的临床效果。结果单用吻合器吻合组发生吻合口漏3例（1．9％），吻合器、闭合器双器具吻合组发生吻合口漏4例（2．5％）；吻合时间分别为（20±3）min和（20±4）min；吻合器平均费用分别为2500元和5000元；均无吻合口出血和狭窄。两组术后吻合口漏、吻合口出血和吻合口狭窄发生率及平均吻合时间差异均无统计学意义（P〉0．05）；两组吻合器平均费用差异有统计学意义（P〈0．01）。结论单用吻合器在中低位直肠癌切除直结肠吻合的吻合口质量与吻合器、闭合器双器具吻合无显著差异，且更经济。

Clinical effect of application of single stapler in operation of middle and low set rectal cancer

Objective To approach the value of single stapler in operation of middle and low set rectal cancer. Methods The application of single stapler in 162 cases of middle and low set rectal cancer was compare to that of double staplers in 158 cases. Results Anastomotic fistula occurred in 3 cases（l. 9 %）in the single stapler group, and 4（2. 5 %）in the double stapler group, respectively. No anastomotic leak,stenosis or anastomotic hemorrhage was found in both groups. The average anastomosis time in both two groups was （20 ± 3） min and （20 ± 4） min respectively （P〉 0. 05 ）. The average cost of staplers in the single stapler and double stapler groups was 2500 and 5000 yuan RMB respectively with the difference being very significant （P〈0. 01 ）. Conclusion The clinical effect of colorectal anastomosis with single stapler was as good as with double staplers, but single stapler was more economical than double staplers. Application of single stapler in operation of middle and low set rectal anastomosis may take the place of double staplers.
